My child has my permission to participate in the after-school Homework Center program at the Watsonville/Freedom libraries.
I have read the rules of behavior and agree that my child will follow these guidelines. I have also read the Internet policy. I understand that the Homework Center computers have Internet access and are for educational purposes only. I agree that my child can use the computers and the Internet under the supervision of the Homework Assistants in the homework center. I give permission for my child to be released from the Homework Center to the Library if his/ her homework is finished.
I will pick up my child from the Homework Center before it closes at 7:00 PM.
